Mobile Phlebotomy Terms, Policies & Disclaimer

Thank you for choosing Gentle Touch Mobile Phlebotomy. We provide convenient, professional specimen collection in your home, office, or another approved location.
By scheduling or receiving services, you agree to the following terms:
1. Our Services
Gentle Touch Mobile Phlebotomy provides mobile blood and specimen collection services based on an order or request from your healthcare provider, laboratory, or other authorized person.
We collect and handle specimens as part of our services. We do not perform laboratory testing, diagnose medical conditions, interpret test results, or decide whether a test is medically necessary.
2. Laboratory Testing & Results
After we collect your specimen, it is sent to the designated laboratory for testing.
The laboratory is responsible for testing, processing, analyzing, and reporting your results. Gentle Touch Mobile Phlebotomy is not responsible for laboratory errors, inaccurate results, reporting errors, processing delays, or other problems caused by the laboratory.
If you have questions about your test results, please contact your healthcare provider or the laboratory performing the testing.
3. Shipping & Delivery
When a specimen needs to be shipped, Gentle Touch Mobile Phlebotomy will package and handle it according to applicable laboratory instructions and shipping requirements, this includes centrifuging if necessary.
Once the specimen is given to a shipping carrier, courier, or laboratory, we are not responsible for problems outside our control, including:
Lost or damaged specimens
Delayed or incorrect delivery
Weather-related delays
Carrier service interruptions
Delays caused by weekends or holidays
Laboratory receiving or processing delays
Specimens rejected by the laboratory after shipment
If a specimen is lost, delayed, damaged, or rejected after it leaves our control, you may need to contact the shipping carrier, laboratory, or healthcare provider for assistance.
4. Laboratory Requirements
You are responsible for providing accurate and complete information needed for your specimen collection, including laboratory orders, healthcare provider information, insurance information, and other required documents.
If a specimen must be recollected because of a laboratory error, you may be responsible for an additional collection or service fee.
5. Your Responsibilities
You agree to:
Provide accurate personal and medical information when requested.
Provide a valid laboratory order when required.
Follow any fasting, medication, hydration, or other preparation instructions from your healthcare provider or laboratory.
Tell the phlebotomist if you have a history of fainting, difficult blood draws, or other information that may affect your collection.
Provide a safe and appropriate location for the blood draw.
Follow instructions provided by your healthcare provider or laboratory.
6. Risks of Blood Collection
Blood draws are generally safe, but they may cause temporary discomfort, bruising, bleeding, swelling, dizziness, or fainting. In rare cases infection or other complications may occur.
Please tell the phlebotomist immediately if you experience significant pain, dizziness, or other concerning symptoms during or after the blood draw.
7. Cancellations & Appointments
Mobile appointments require travel and advance scheduling. Please provide reasonable notice if you need to cancel or reschedule. We waive all cancellation fees.
Gentle Touch Mobile Phlebotomy will charge a fee for no show appointments, or travel fees according to our current fee schedule.
8. Responsibility & Limitations
Gentle Touch Mobile Phlebotomy will make all efforts to provide safe, professional specimen collection and handling.
Nothing in these terms is intended to waive or limit any responsibility that cannot legally be waived or limited under applicable law.
